Just wondering, since you've gone to the effort of incorporating John and Gillian, would it make more sense to put them during the Massacre? the Doctor is missing for a long time in that story with no explanation of where he is.
What about this as a theory - the Doctor is in a Parisian garden when he's time-scooped for Five Doctors. He then leaves Gallifrey in his TARDIS and gets sucked into an Unbound universe where he adventures for a while with John and Gillian. Eventually his is rescued when the time Lords scoop him for his appearance in Three Doctors (the extra energy required to get him from another dimension could be why he got stuck in that capsule on screen), and this time they put him back in his real place in time and space so he can rejoin Steven.

I'm just about finished with The Fifth Traveller, and I believe I can firmly say that it takes place between The Web Planet (TV) and The Dark Planet (Audio). There are multiple cases where the TARDIS crew reference The Web Planet and The Romans, and at one point Ian specifically lists off that they had most recently been to Vortis, China, Byzantium and Rome.
I didn't really notice any clues for the previous Early Adventures story, The Age of Endurance, so I just put it after Domain of the Voord.

I've just finished listening to The Sontarans. Aside from being a really, really good story, it also offers more than a couple hints at its placement. Sara notes how the Doctor "always knows what to do" in a crisis and refers to An Ordinary Life as occurring "a lifetime ago," probably placing it sometime after the events of Home Truths. The final scenes of the story actually lead into "Volcano" and the Daleks' first test of the Time Destructor.
Katarina in the Underworld is a little difficult to place due to it, well, taking place in the afterlife, but I think it fits rather snugly before The Little Drummer Boy. The Doctor finds himself within the Underworld with Katarina without explanation in the former story and in the latter, he's fast asleep as the TARDIS materialises in Victorian England during Christmas time. The Anachronauts seems to follow almost directly from The Feast of Steven with Sara and Steven indulging in a bit of bubbly, so the stories appear to play out something like this:

E is for... Susan’s telepathy powers are getting stronger and she’s quite familiar with them, so it’s after The Sensorites.
Daybreak Features Ian, Barbara and Vicki. I’d probably put this quite late along with the Ian/Barbara/Vicki stories from the first set, as it seems like a more mature Vicki. She encounters historical figures from her past but Barbara and Ian's future and learns that history isn’t always recorded truthfully.
The Vardan Invasion of Mirth Easy placement. Steven already knows about the Vardans, so it’s after The First Wave, but it’s just him and the Doctor so it’s before the Massacre. Sticking it in after O'Tannenbaum works fine.
The Crumbling Magician Between The Bonfires of the Vanities and The Plague of Dreams, like all the other EU Ben and Polly stories (except Falling). Ben mentions that the Doctor has been complaining about getting old a lot recently and it’s clear that he and Polly have been with the Doctor for a while, so I’d put this right before The Plague of Dreams. Also they may or may not take on a new companion played by David Warner, we’ll just have to wait and see in the next volume if they keep him on.

Don't ask me to justify it because I don't remember the logic, but I've currently got Hunters of Earth slotted in between Quinnis and An Unearthly Child.

In Quinnis, the Doctor decides to put down roots somewhere for Susan's sake. It’s also mentioned as being 4 or 5 journeys ago in The Edge of Destruction. Hunters of Earth takes place after they’ve settled on Earth. It’s set in October 1963, a month before An Unearthly Child.

I think Hunters of Earth comes first as it’s set in October 1963, whereas An Unearthly Woman takes place the first week of school after half term which likely means early November if half term has fallen over the Halloween period as it generally does today.
