What a great question!
I’d say the best cliffhangers are something unexpected that further the story. Reveals can be effective, but only if it’s not spoiled or obvious beforehand and it’s not something or someone that’s been done to death.
I think the worst are ones that involve the Doctor and/or companion getting killed. I’d say Big Finish are often guilty of this, but only because, on the whole, they’re dealing with characters with endings we are already familiar with.

Thought... BF should do some episodes with no cliffhangers. Just do a straight hour or two of writing.
I do think there are some that works. I'll have to think about them, possibly by reading my iTunes list. The trouble is that a main character cannot be threatened for it to work for me. I know it'll happen a lot, etc., but it's expected. A cliffhanger that works is.....I don't know, it's like a number of the scene-changing pauses in a BF episode like The Peterloo Massacre function. Something very bad approaching should be palpable, but that's hard to achieve.
Too often they feel forced. I would like to see what a number of releases sound like in stories that aren't structured around cliffhangers on a schedule.
