Jago & Litefoot. Does Torchwood count? If so, I'd put it second. Also, UNIT (The New Series).
Beyond that, what are we defining as a "spin-off"? I'll assume it's "Doesn't fit the 'Doctor, companion, Tardis travel' format." And are we going for ranges or single-releases?

Depends on what sort of story people are interested in, I suppose.
Jago & Litefoot for hilarious fun. Dalek Empire or Cyberman if you want something upsetting. Gallifrey if you want something so cerebral it may break your brain.

Jago & Litefoot, obviously. Best to start with The Mahogany Murderers since it does tie into Series 01 (and it's also a brilliant story).
I'm counting Torchwood as a spinoff, so if you haven't checked it out, most of the single releases are pretty self-contained and easy to jump into.
I also really, really like The Diary of River Song. It's a case where I think the second set is better than the first, but the quality of both sets is pretty high. Plus, if you're a bit uncertain about jumping into a range without the Doctor, they pop up often enough in these two sets to ease you into the idea whilst also never taking the focus off River.
For Bernice, I've only really listened to The New Adventures of Bernice Summerfield (though if they ever release the single releases on download, I will definitely be buying) and I'm not really sure I'd consider The New Adventures of Bernice Summerfield a spinoff series or not. But Bernice Summerfield does take center stage for most of these stories and they're really brilliant - ESPECIALLY the third volume!

Most of the spin-offs from the Doctor Who universe that Big Finish have done are worth listening to, but my favourite range is Dalek Empire. The first series especially really delivered when it comes to great story telling with characters you really believe in.
Cybermen, Jago & Litefoot, Counter-Measures and Gallifrey are all recommended ranges too. They are all very strong sets of stories. There is a spin-off range for everyone.

Jago & Litefoot & Strax is a great first step into spin-offs for any fan. An affordable full-length story on 2 CDs, brilliantly melding the classic and new series. And it's one everyone should hear: hilarious.
Then you will probably want to explore J&L, starting with the original Companion Chronicle, 'The Mahogany Murderers'.
And if you are a classic-series fan, then a great spin-off (and a place to start discovering excellent Bernice Summerfield) is the New Adventures 2 box set: 'The Triumph of Sutekh'. Yes, him. Plus the Seventh Doctor and Ace, in a superb four hour epic set of stories.

Gallifrey 1 and 2 If I were to pick any one or two of the above what are your recommendations ? I an only speak for Gallifrey as it's the only one I've heard, so I would absolutely recommend that! (Though you should also grab Series 3 while you're at it, as you'll regret it if you don't )

I enjoy all lot of it and it is very versatile with the stories that it tells. Something for everyone. Also Lisa Bowerman is just a joy to listen to.

I love Gallifrey as well, but it doesn't stand on its own as well - it definitely needs knowledge of specific Doctor Who episodes/characters/concepts, whereas Jago & Litefoot feels like a spin-off of a single story and barely even requires knowledge of that story.
The series stars a fabulous double act (and Ellie and Quick to round out the cast). It's also just brimming with Victorian atmosphere, creative uses of classical monsters (you got your vampires, your aliens, your ghosts, etc) and such wonderful writing. Oh, the writing. Oh, the alliteration! It's beautiful. My absolute favorite - and I'm still gutted that we've lost Trevor.
As for others that I've heard, Dalek Empire was interesting, but it takes some time to get going. If you're into a slow burn and a solid gritty story, you'll like it. Counter-Measures is a lot of fun in a similar period-piece creative-monsters way as Jago & Litefoot, but of course, the characters are not drawn as well in their original appearance, so while I do enjoy them, they don't start off with as big of a bang.
And like I mentioned, Gallifrey. It's basically a time-warping political drama with Leela, Romana, the Inquisitor from Trial, Braxiatel (from the Virgin novels) and some great original characters.

I have the first season of Jago and Litefoot . yet to try it out . Dalek Empire was simply brilliant . one of the best i've heard till date.
