Mine would be a small moment in The Time of the Doctor. The Doctor loses his leg as a result of the Weeping Angel grabbing his ankle; it punctures the skin and turns it to stone. He attempts to regenerate a new leg and no energy leaves him to do so. He simply can’t regenerate. And that’s when he realises he’s at the end of his regeneration cycle.
It’s not a dramatic change. The Doctor losing his leg and it being replaced with a wooden one was in the original script and picked up on in the Tales of Trenzalore short story collection; it was due to be the reason the Doctor had a walking stick as a result of Matt Smith‘s knee operation. However, it’s a quiet moment where the Doctor finally realises he’s at the end of his life as he never quite knew the impact of the Metacrisis or giving energy to River, etc. And it would have nipped knowing references to the Doctor being aware he’s on his last regeneration; that doesn’t sit well with me.

The Doctor in footy pajamas but an incarnation you would not expect to be like 3, 6,9 or 12. Their Companion insisted! They protested but end up liking how cozy they are!😁
9 in formal wear!
12 giving more concerts and getting into dancing around while he sings and plays the guitar more than you would think!
The Doctor and companion(s) swimming through the stars as though it were an ocean. Possibly even in swim suits and with pool toys. Maybe with the Doctor showing off by diving of of TARDIS!

That's in my headcanon as well and has been since the episode (although I hadn't thought of the Weeping Angel). I still think it can fit - the Doctor is pretty sure he's on his last life but he can't help but hope.

The Doctor and a companion (companions) have to spend the night together camping around a fire and it’s both sweet and awkward with singing around the fire, bonding time and trying to figure out sleep areas and possibly dealing with snoring.
Clara’s dad and gran meeting 12 and wondering how she knows him ( not realizing that they already met him when he was 11.)
The Doctor wears something ridiculous (they would never normally wear) a companion got them to make them happy, or vice versa. Like imagine 12 in a onesie Clara got him (perhaps a footie one with a hood and little animal ears) or Sixie wearing a rainbow spandex workout outfit and leg warmers Mel bought him.
