Are we doing visual only? Because I'm throwing The Red Lady in here as top scare material. The Red Lady is terrifying. Unfortunately, they gave us an explanation/origin for her which made her less frightening in my opinion.

Humans, every time. Midnight, Live 34, Countrycide, The Massacre, The Witch Hunters, The Peterloo Massacre... I'm sure there's a bunch of other stories I'm forgetting, but there's really no contest!

I'm with rran on a couple. The creepy "monster" from Midnight that feeds on anger and hate and steals your voice. The face The Doctor makes when he is taken over by the thing and left impotent, unable to use his greatest weapon. That was scary as hell. The Vashta Nerada are up there for me. The Cybermen because of the body horror and the desperation of the humans that thought they were a good idea. I agree that the Red Lady was scary when listening to that story for the first time. But I think my all time scariest/favorite is The Wirrn. Gets me every time. It's the body horror mixed with giant insects...terrifying.
