So this is one of the oldest gags about this franchise: along with wobbly sets and screaming girls in miniskirts, the fastest way to goad a Whovian is to call the Doctor Dr. Who, or hear someone else do so.
But let's unpack that stereotype for a minute: do you actually care if anyone calls the Doctor Dr. Who? Has it ever bugged you in any way?

No. If Peter Capaldi the actor who played my favorite incarnation of the Doctor, goes around proudly and happily referring to himself as Doctor Who why would I let it bother me when other people call the Doctor, Doctor Who?😁

Calling the Doctor "Doctor Who" doesn't really bother me but calling them "Dr. Who" does. Never abbreviate!

As has been said many times before (not sure where I heard it first), Doctor Who is the name of the show, the lead character is Doctor Who's Monster.

I'd say, it depends on the context. I've a preference for the prefix over the suffix myself. To paraphrase the Fourth Doctor, there may be a doctor in the room, but they're the Doctor. The definitive article, you might say. There's a wonderful mystique to that. But... I do like Doctor Who as a bit of stylistic shorthand for stories sourced from other continuities. Carte blanche for Cushing, for instance, or TV Comic's Doctor Who and his grandchildren, John and Gillian. It helps change the mental gear box towards a different set of narrative rules.

As all true fans know the character has been referred to as both in the credits over the years. True fans don't care which is used to describe them.

Dr Who was Peter Cushing's eccentric English Inventor in the Amicus Dalek films The Doctor is the assumed alias of Alien character in the TV series Doctor Who is the name of the series
The War Machines featured an anomaly that Peter Moffatt played upon with Missy claiming his name was Doctor Who, probably to wind up those for whom this is something to get worked up over.
For myself, I use the Dr Who abbreviation all the time and also refer to the character/actors verbally as 'Doctor Who'. I have a few books and Jigsaws from the 60s/70's where it is labelled 'Dr Who', so that's legit enough for me.
So in answer to the question. No. And we forgive those who do. Providing its a one off.

Nope. It's used frequently in the end credits, Peter Cushing uses it, many of old annuals (and comics, etc.) use it, many of the actors and writers use it outside the show... I'd probably not call them that inside the series itself (even though WOTAN does!), but there's nothing wrong with adding it to the list of the Time Lord's aliases. (It's not like Doctor is their real name either!)

(It's not like Doctor is their real name either!) Fits nicely among calls for Zeus, Mr Snowman, Merlin and the mighty Qui Quae Quod (and, at the risk of having too much fun, the mysterious Oho, Delphinian vortex-slang for "the question begins with the answer" ). Their real name, 'tis lost to the deepest recesses of Time and thoroughly unpronouncable to the human tongue. The closest we got for ages was ΘΣ *cough* *clears throat* Sorry. I mean, "Theta-Sigma". A... somewhat infamous grade of mental and physical acuity for a graduating member of the Time Lord Academy (a bit like walking around with everyone calling you "D+", ouch). That, and Dr John Smith. Interesting thing about all of those, they don't mind being known circumspectly by an alias for a time, but really start to squirm when it starts looking like a real name, rather than a title. There's some fundamental part of the Doctor that prefers to be unidentifiable. Even now. Interesting, that.

Doesn't matter that much to me, I don't think. If aussiedoctorwhofan's boy wants to call him "Doctuh Who", who am I to correct him? Besides The important thing is that he knows the name of the show :-) If it's a mistake, sometimes it's an easy one to make as well. Before my memories of the show started coming back to me, I wrote four lines of a Dark Shadows-Doctor Who cross-over where naturally The Doctor gets put in Dr. Hoffman's asylum because he insists he's from the planet Gallifrey (right before she mentions that it's a common delusion for patients to think they're doctors) and naturally I didn't know better than swapping in "Doctor Who" for "The Doctor". What can I say? At the time I couldn't even remember who Sarah Jane was and I was easily taken in by things like "Doctor Who and the Pirates" or the Nev Fountain Fifth Doctor story where The Doctor is writing adventures of "Doctor Who" and etc. It's easy enough to get straightened out though. I imagine usually some kind person will lean over and whisper "he calls himself The Doctor" like they were telling your fly was unzipped, and that's that.

For me its The Doctor I tend to think that the reason WOTAN in The War Machines calls him Doctor Who, is because he's picked up the message through time from the Crack

Did anyone in-universe refer to the character as "Doctor Who"? All I recall ever hearing him referred to in-universe as is "the Doctor", or if someone addressed him directly "Doctor".
"Doctor Who" is the name of the show, and the closest I ever recall to an in-episode reference to it was in the form of the occasional joke.
"Who was that man?" "Oh, that was the Doctor." "Who?"
...or when he's introducing himself...
I'd find it really odd if I was ever listening to an audio and one of the characters referred to him as "Doctor Who", as if it were his name.
