This sort of thing has never sat well with me. Does it mean he'd also "trick history" to save say Loraine Allison etc.? or would he let her die as history records?

I’d imagine when it comes to changing history, he’d do it on a case by case basis. What with fixed points and all. Now whether it’s entirely ethical?
Hard to say, despite their empathy towards humanity the Doctor (despite what recent alterations to the shows mythos might say) is from a race who can see the bigger picture when it comes to the delicate intricacies of Time and Space. More so than any human can possibly comprehend.
