I think you could make a case that from the moment Capaldi was announced as the Doctor, with that Hartnell tug he gave the jacket he was wearing at the moment, that in his first series he was doing kind of a modern riff on the 1st Doctor.

I missed out the 10th (second)regen as there wasn't a change in body, as for the movie could have called doctor who and coat

The show would have ended very quickly because a five year old would not have been able to give the role sufficient gravitas and would probably have been even worse than Hartnell at remembering lines.

I don't think there'd be much difference.
A more interesting question would be 'What if Jodie Whittaker had been the First Doctor?' Would we have had 55 years of white British women in the role?
