The Suffragette scene in The Suffering always gives me that gut punch. That's the number one heartbreaker I think of. The Suffering is underrated.

Absent Friends by John Dorney, for sure.

I wouldn't say I've felt anything like "heartbreak", but will significant emotional oompf do?
To the Death The Girl Who Waited Amy's Choice The Peterloo Massacre Prisoners of Fate Absent Friends
I'm sure I've forgotten some. There are way more along the general lines, like The Settling or The Neverwhen, or much of Dalek Empire in terms of what is going on overall (and plenty more such episodes)

Seconding The Peterloo Massacre. Not only incredibly sad, but also the only pure historical story I would describe as genuinely scary.

If we're strictly speaking Doctor Who it's easily Absent Friends & A Full Life for me. But throwing spin-offs into the mix then The Sky Man (War Master 1) and Corpse Day (Torchwood Monthly) would be my choices! Corpse day? OOOH!! BF let me get that for my freebie with subscription to the main range. I'm even more excited about it now that you have said it's emotional. As for me..... Peri and the Piscon Paradox is obvious. I am also a DV survivor so it was particularly relevant for me. Lights of Skaro and indeed the full box set it concluded was full of heart pulls. Life During Wartime has a story by Rob Shearman about Peter that just is wow. I listened to Jago and Litefoot revival after the passing of Trevor Baxter. George gives this beautiful evocative description of the Doctor and it was like he was describing Mr. Baxter. I was full of tears. Patient Zero. India Fisher was such a wreck that she insisted on multiple takes for one of her scenes. And she made the right choice. It killed me inside and I have never wanted to hug someone more. And as a bonus you can hear the original first take and.... well I udnerstand why it was important for India to rerecord. Speaking of Charlie.... Scherzo and The Condemned. The Charlie and Sixth Doctor Adventures was my first purchase through Big Finish after...<ahem> hearing most of the 8th Doctor stories(which I now own on cd) The condemned is a great tale but more than anything it is about you as the audience. I wouldn't call it an emotional story by itself but if you have listened to all the Charley and 8 stories it's really emotional because of what Charley is going through in her adventures with this very different Doctor. Her entire story arc is pretty damn emotional to tell the truth. Her and Lucy make me cry so damn much. I love Helen and Liv but soemthin about Lucy and Charlie just pulls at those heartstrings to a terrible amount. Other Lives was really emotional too. As for Lucy.... Death In Blackpool. To The Death/Lucie Miller and although it isn't mentioned much, Orbis. Orbis was phenomenal in that I didn't care about Orbis when it was over. Lucy is put through the emotional wringer so badly in that story and it always tears at me.

Can I throw in The Curious Incident of the Doctor in the Night-time? Just beautiful, and I made the mistake of listening when I’m driving (which I wouldn’t recommend, as your eyes might become blinded by the tears).

In Gods and Monsters, "I'm coming, mum!" or something to that effect, even if Afterlife brought him back. Also the Sad Evelyn Tetralogy of Jubilee/Pirates/Lazarus/Arrangements - they all have their moments, especially when enjoyed in order. And Death in Blackpool - followed by a beautiful payoff at the end of The Resurrection of Mars when "In the Bleak Midwinter" plays. I can't hear that song now without thinking of Lucie.
