I think the OP question is a bit vague: are we talking trigger as in 'complaint (which is probably a more fitting word)' or trigger as in 'something that is uncomfortable/upsetting to watch/listen to?'

Nucleus is correct, the term really means that you have watched/heard/read a piece which makes you uncomfortable, often for extra-textual reasons. So its fair to say some were triggered by Stranded 4 hittin too close to home. Last night I was a guest on a Youtube show discussing Amicus films and I warned people before I mentioned a very horrible rape scene. Now..these films are full of murder and thats fine, which may seem hypocritical, but the depiction of rape in a very real and serious way is totally different from a Werewolf murder.
It is also a term co-opted by people who are out to offend, sadly. They will use "Are you triggered?" in a belittling fashion, implication that you are thin-skinned. Nonsense of course but part of the modern warfare over words.

Anyway, I don't think I've ever been 'triggered' by any Doctor Who (or broader Whoniverse) TV episode, or audio. It's just entertainment, and I'm perfectly capable of living with my emotional issues without entertainment of all things triggering me.
