Gallifrey Series 4 has a pretty funny outtake where Lalla Ward loses it a bit.

Frankenstein also has almost ten minutes of outtakes, as I recall...
I try to include them wherever I can... just because!
(Suffice to say, Dorian Series Five has a fair share coming up too...)

I agree that bloopers, and cast being more casual and relaxed, are great things to hear and I wouldn't mind hearing more of them, but I have to say that I'm still really impressed even with the usual "so and so wrote a great script" sort of thing because it seems like more or less invariably, cast members really do have some keen and insightful things to say about why it is a good story, rather than just heaping idle praise on the authors. It often seems to me like the performers are genuinely invested in their characters and the stories and I think often enough might make the same kind of comments that fans might make, which is very gratifying as a fan.
Just a comment I've been meaning to make for awhile and I finally found the thread the comment belongs to. :-)

There are little short bits of outtakes on many of the CD extras. They're just usually not collected into a separate group, nor run for any length, but I can recall quite a few with Tom Baker going on past the point where the scene ended, or where Nick Briggs will give an unusual line in a Dalek voice.

I listened to the extended extras for The Peterloo Massacre yesterday and thought it was really interesting and insightful.
