I'd say even before that - if you watched The Horns Of Nimon as a newbie with no concept of Doctor Who, you'd think Romana was the hero and The Doctor her bumbling sidekick more concerned with the dog. And I'm not sure you'd be wrong... I think a comedy story with an early Romana II and K-9 could work, where they're trying to do something outside of their comfort zone, like renting a car or going grocery shopping. Why do I suddenly have an image of Romana I buying/selling shares and getting accused of illegal insider trading because she's too good at it?

Definitely Three/Liz and I'd also say Three/Sarah Jane.
Four/Romana II/K9, particularly by Season 18 (although K9 was pretty much written out by that stage).
Certainly with Big Finish fleshing out a lot of companions, we've gained some more "competent" TARDIS teams.
Definitely Five/Nyssa is a very competent team, Old Sixie/Evelyn was as well and I don't think you could argue with Seven/Ace (I'd say perhaps - controversially - earlier in the range in stories such as The Fearmonger, Colditz, The Harvest and LIVE 34).

Weirdly, the first one that came to mind was Five and Nyssa. There's no drama between them (any team with Ace!), they're both scientific minds and there's a genuine curiosity for exploring the universe that keeps them moving.
But Seven-Chris-Roz feels very true as well. And Six-Constance. And Four-Romana-K9.

I'll echo what's been said about Four and Romana II. Same for the Fifth and Nyssa, Three and Liz. I'd add Eight, Liv and Helen to the list, as well as Six and Constance or Evelyn.

Definitely the Eleventh Doctor, Amy, Rory and River Song. The Pond family. Seeing them all work together in Series 5 and 6 was great and it's a shame we did not see more of that in Series 7 in their last episode together.
The Fifth Doctor, Adric, Nyssa and Tengan. One of my favorite teams and worked well together in their first trilogy in Big Finish without being left out (cof Turlough cof)
The Twelfth Doctor, Bill and Nardole, have few episodes but together they really are a astonishing team.
The Seventh Doctor, Ace and Hex. A pity that an overcomplicated saga has ruined such a promising team.
The Ninth Doctor and Jack Harkness (Rose does not count since it was reduced to a damsel in danger or a plot device like the Bad Wolf)
The First Doctor, Ian, Barbara and Vicki.

Fourth Doctor and Romana(II, more experience and a very determined personality!) and K-9. What a team: the Time Lord and Time Lady who walk in eternity with their technological 'Swiss Army Knife' of a robot dog; for all-round competence there is no competition.
