At the end of Empress of Mars, Missy asked the Doctor 'Are you alright?'. I've seen theories online that the Doctor is suppressing a regeneration, and The Lie of the Land wasn't a faux regeneration.
I think those theories are right. I think the Doctor suppressed the regeneration to convince Bill that she hadn't killed him and to save her soul, so that she wouldn't be blaming herself for killing his current incarnation. He did it to protect Bill from herself, and is keeping it secret from her and Nardole that the regeneration energy is slowly killing him inside.
When you hold back regeneration energy, I think it's possible that it could lead to radical changes as the regeneration energy has to go somewhere. I think it begins changing his genes inside the body, and the longer you hold it back the more drastic the change is. That's what Moffat means about the regeneration being different this time round.
Now we have seen the Doctor do it before in The End of Time (resulting in the Doctor regenerating into a much younger body) and The Time of the Doctor (Doctor in a much older body) but both were only for a short period of time. If this theory proves correct, the Doctor will end up holding it back for three stories in total (and most of Lie of the Land) before letting the process continue at Christmas. That would mean a very drastic change. I doubt they will go older, and they won't go younger than Matt Smith (although I think it's fair to say the next Doctor will be a lot younger than Peter Capaldi).
The most drastic change I can think of is a change of gender, and that would tie into Chibnall's comments about Series 11 being bold and risky. If it does end up being a female Doctor then this theory would give those who are opposed to the idea a canonical reason for why the Doctor is suddenly female after 54 years of being male.

See, I thought the unusual thing about Missy asking if the Doctor was all right, was because it MISSY asking about someone's welfare.

I get what you mean, but if we are going by theory, it wasn't that long after the "Faux" regeneration that they went to Missy. But more time has passed between then and when she's in the Tardis, so to someone who's had Massive regeneration problems themselves, it's something they would notice

See, I thought the unusual thing about Missy asking if the Doctor was all right, was because it MISSY asking about someone's welfare. To me it sounded as though she was mocking him. I believe the 'good' thing is a facade and Missy is playing the Doctor.
