I won't pick Night - it's my favourite Nu-Who thing ever but it's a minisode prequel, not a special. If we count that, there are LOADS more that we'd need to count. Day, too, seems too easy a pick. Massive fannish fun but it's impossible for me to seperate it from the experience of the live big screen experience, a packed cinema, going to the pub then home for the Fiveish Docs...that's in a league of it's own. Even picking a regen seems somewhat of a cheat as they to tend now to be big epics full of lore and whatnot - comparing that to Madge losing her Spitfire pilot husband in WW2 isn't really fair.
So of the actual annual specials? A Christmas Carol for me. It's the perfect use of the 11th Doctor and amazingly finds a way to give the Dickens story a somewhat fresh take. Michael Gambon was a real casting coup and Murray's score was never better. Just lovely all round really.
