In Iain M. Banks' Culture novels, citizens of the Culture are able to swap genders simply by triggering a biofeedback mechanism in their dna. They can do this by merely thinking really hard about it. One minor character spends some time in a "neutral" state because they can't decide, two characters get each other pregnant and one places the foetus into stasis within their own body in order for their partner to "swap" and catch up so that they can give birth together.
Perhaps gender-swapping is a conscious choice on the part of the time lords: they might do so once or twice in their lives to "experiment" and see what it's like on the "other side," as the General seems to have done.

Yes genius indeed. I had expected and spent many a minute wondering who would be cast as The Twelve and I confess that despite the new novelty of gender swapping available to Time Lords I never considered that it would be a woman. Let alone an evil Miss Marple! God, I love Big Finish's dark sense of humour. This time, Miss Marple's doing the murders! Given the nature of the Eleven/Twelve, it's not impossible they have a story where she solves her own crime and catches herself. "One of us is guilty - one of us in this very body..." (OK that line's more Poirot!)

just as Romana [has] been exclusively female so far. I still like the fan-theory that "The General" is, in fact, Romana Ok I hate that theory. Mainly because in both of her/his appearances the General is a complete a-hole and is just standard Moffat woman #27 and not Romana, the noble Time Lady that the show presents. I haven't listened to Gallifrey so if I've got stuff wrong about her development let me know, but I don't think the General works as Romana.

The General is also on their 11th incarnation, I believe, which would make Romana extremely unlucky in addition to requiring her to have a third sudden switch in career. It’s not impossible, but I don’t think it’s very “neat” and I don’t think it adds anything to either character. I’d rather see some Big Finish appearances of earlier Generals than try to connect two unrelated characters together. And, frankly, I don’t think we’ve seen enough gender-switching regenerations. The only reason we’ve seen such a burst of them lately is because it was previously ground Big Finish and the show were nervous of covering due to the controversy (or, for some writers/producers, ground they did not want to cover). Now that we know Time Lords can switch gender, there’s no reason not to show it.

Time Lords also have the potential to regenerate into other species (re: Circular Time), so there's likely a lot more capacity for change than the Gallifreyans let on. The limitation might be cultural, rather than biological. vs.
